INCOMING FRESHMAN PHYSICALS

• A physical examination dated after August 15, 2018 and a current immunization record must be submitted for every incoming 9th grade student no later than August 1st, 2019. The State of Illinois mandates that the health physical must be submitted on the official Illinois Department of Public Health Certificate of Child Health Examination Form. The IHSA Sport Physical form cannot be accepted. Once your 9th grade student has submitted a physical examination and immunization record to the Health Office, and if they wish to participate in a sport, a separate sport’s physical will not be needed. Students who are not in compliance by the first day of school will be unable to attend class.
